Built as a customs house, the Old Custom House now operates as a 23-room inn on South Quay in Padstow, directly overlooking the working harbour. The listed building stands on the quayside; rooms occupy the first and second floors, accessed by stairs. All rooms are en-suite and face either the harbour or the town. Each includes a double bed, contemporary bathroom, Wi-Fi, tea and coffee station, and hairdryer. Cosy Doubles serve as the entry category. Breakfast is included in the room rate and runs each morning; the continental selection is self-service, and a hot menu is available to order.

The ground-floor restaurant serves breakfast, lunch, and dinner. The menu combines pub classics with seasonal specials drawn from local ingredients. The Camel Trail, an 18-mile traffic-free route for cyclists and walkers, begins in Padstow. Boat trips, fishing charters, and the passenger ferry to Rock depart from the harbour.
